How to fill Account opening form of Punjab and Sind Bank
To apply for account opening one needs to visit a Punjab and Sind Bank where he needs to fill up account opening form. Attach zerox copies of the documents i,e Aadhar and PAN Card. Then he needs to submit the form and get it verified with the bank officials. After the form is verified, the customer is allotted account number after which the customer can start making the first deposit. The customer receives the pass book and cheque book within few days of account opening.

Entries to be filled in the account opening form
Firstly, one need to paste a recent passport size photograph along with it he needs to draw his signature besides the photograph
Personal Details: The applicant needs to write his full name in the format of First name Middle name Last name
**In case of Joint account second applicant should write his name on 2nd Applicant coloumn
Name of Father: The applicant needs to write his father name, in case of women name of husband should be written
Gender: Applicant needs to Choose the gender
Date of Birth: Applicant needs to write his date of Birth on this coloumn
PAN: The applicant needs to write his PAN card number on this coloumn
Communication Address: One needs to write his residential address on this coloumn.
Staff: If the applicant is a staff member of Punjab and Sind bank then he needs to specify his PF Code, otherwise select NO
Minor: If the applicant is a minor then he needs to write his date of birth, name of the guardian name representing him or her, relationship with the minor,address of the guardian
Account Opening operations: choose whether the account is single of Joint account.
Deposit details: Write the amount you are depositing to the bank as initial deposit. Choose between cash or cheque, mode of depositing the amount. In case if you depositing the amount by cheque then one needs to write the name of the bank it is drawn, cheque number and the date of the cheque.
Request for seeding of Aadhar number: If you want to link the bank account with the aadhar number then one needs to write this aadhar number in this coloumn.
Tax Residence declaration under FATCA-CRS: Choose the first coloumn stating that you are a resident of India. If you are resident of another country choose the second coloumn.
Occupation Details: Here in this coloumn you need to provide the informations about your business or job like type of business, type of job, source of funds, Gross Annual Income, etc
Internet Banking Service: Select I wish to apply for PSB Internet Banking Service
1st Applicant: Write the email address
Mobile Banking: Select I wish to apply for Mobile banking service. One needs to write his mobile number
SMS alerts: Select I wish to apply for SMS alerts service. One needs to write his mobile number
ATM Card/Debit Card: Select I wish to apply for PSB ATM DEBIT CARD. Select for Cheque Book facility
Name to be printed on the card: Write the name one needs to be printed on the card
Form No 60/61: If one doesn’t have PAN card then he needs to fill up this coloumn. To fill this coloumn one needs to Just write his full name under Applicant name. Then he needs to just write the amount of transaction. Then one needs to select No, for statement are you assessed to tax. The applicant needs to submit documents in support of identity and address.
Nomination: In this coloumn one needs to provide informations about the nominee that one wills to nominate in the account. To fill the coloumn one needs to provide the name of the nominee along with his age and the relationship with the depositor and the date of birth.
Signature / Thumb impression of the 1st Applicant: Applicant needs to draw his signature in this coloumn.
Place: Write the city name
Date: Write the date when you submitting this form
Signature: Here the nominee needs to sign in this coloumn as 1st witness or any other person who already have account in this bank. The said person also needs to write his name and this address.
**The form is duly completed, one needs to attach one zerox copies of Identity and the address proof.
