Entries on the Top part of Andhra Bank withdrawal form:
Branch: Write the name of the Andhra branch from which you are withdrawing the money.
Date: Write the date when you are making the withdrawal
Not Negotiable/Not Payable in Clearing: Select Cash
Name of Account Holder: Write the name of the account holder or name by which the acount holder maintain his account.
Account Number: Write the dedicated bank account number.
Please Pay self/Your self for the purpose of: Quote the reason for withdrawing money from your account.
Signature/Thumb impression of the account holder: The account holder needs to put his signature in this place. The signature should be the same, he submitted to the bank at the time of account opening.

Cash Denomination: In this column you have to mention the denomination in which you want your amount to be withdrawn. You can receive your said amount in denomination of 2000, 500, 100, 50, 20,10 and 5 ( if available )
No: Write the total number of denomination that makes the whole sum you are withdrawing
Amount: Write the amount you are withdrawing from your account.
Pcs: Write zero as paise
Account Number: Write the bank account number.
Name of Account: Write the bank account name.
